Arranging Care

Arranging care

What to expect when arranging care.

Finding care for yourself or someone close to you can feel like a big step. You might know exactly what support is needed, or you might simply know that something needs to change.

Either way, you don’t need to have all the answers before getting in touch.

We’ll take the time to understand the situation, explain the options available and help you work through what happens next.

Starting the conversation

It starts with getting to know you.

You can contact CM Community Care if you’re looking for support for yourself, a family member or someone you care for.

We also work with local authorities and other professionals arranging or referring care.

Your first conversation is simply an opportunity for us to understand what’s happening, what kind of support might be helpful and what’s important to the person who will be receiving care.

You don’t need to know which care service you need before you call.

How arranging care works

01. Let’s talk

We’ll start with a conversation about the support you’re looking for. We’ll want to understand the person’s current circumstances, routines, preferences and what they’d like help with. It’s also your opportunity to ask questions and understand whether CM Community Care is the right fit.

02. Understanding your needs

Where appropriate, we’ll carry out an assessment to understand the level and type of care required in more detail. We look beyond practical tasks to understand how someone lives – what they can and want to continue doing independently, what’s important to their routine and where additional support could make everyday life easier.

03. Creating your care plan

From there, an individual care plan can be created around the person’s needs, choices and routines. It provides carers with the information they need to deliver appropriate support while helping make sure care remains centred around the individual rather than simply a list of tasks.

04. Introducing your care

Once everything is agreed, we’ll arrange for care to begin. Wherever possible, we aim for continuity and familiar carers, giving people the opportunity to build trust and helping our teams get to know the individual they’re supporting.

05. Reviewing care as life changes

Care isn’t something that should be set once and forgotten. Needs, routines and circumstances can change, so support can be reviewed and adapted over time to make sure it continues to reflect what the person needs.

Familiar faces

Because who provides your care matters too.

Inviting somebody into your home is personal.

That’s why we understand the importance of trust, familiarity and continuity. Wherever possible, we aim to provide consistent carers who can get to know the person they’re supporting – their routines, preferences and the little things that make a difference.

It means less explaining and more familiarity. More opportunity to build a genuine relationship. And care that can feel like part of everyday life rather than an interruption to it.

That’s Care beyond care.

Keeping people connected

Care works better when we work together.

Families and those closest to someone can hold a huge amount of knowledge about their routines, preferences and history.

Where appropriate, and always with the wishes of the person receiving care at the centre, we work alongside the people involved in their support.

We’ll keep communication clear and provide opportunities to discuss how care is going, raise concerns and tell us when something has changed.

Different circumstances. Different routes to care.

Home care can be arranged and funded in different ways.

Some people receive care arranged or funded through their local authority, while others arrange and pay for private home care themselves. In some circumstances, other funding routes may also apply.

If you’re unsure where to begin, you don’t need to work it out alone. Our team can explain how CM Community Care works and point you towards the appropriate next step for your circumstances.

Care that changes with you

Support for today, with tomorrow in mind.

Someone’s care needs can change gradually or unexpectedly.

If circumstances change, we’ll work with the person receiving care and, where appropriate, those involved in their support to understand what’s different and review the care being provided.

Our aim is to provide as much familiarity and continuity as possible as support changes – because needing more help shouldn’t mean feeling like you’re starting all over again.

Frequently asked questions

No. You can start by telling us about your circumstances and the support you’re looking for. Our team can then help you understand the next steps.

You can contact us to discuss care for someone close to you. How care is then arranged will depend on the individual’s circumstances, wishes and any other people or professionals involved.

CM Community Care works with local authorities as well as people arranging care privately. The route into care will depend on individual circumstances and local arrangements.

Wherever possible, CM prioritises continuity and familiar carers. We can’t promise that the same individual will attend every visit, but we recognise how important consistency is to building trusted relationships.

Care can be reviewed as needs and circumstances change. Where additional or different support is required, we’ll discuss what happens next.

Our teams support people across a number of communities in the region. Visit our Locations & Coverage page or contact us to check current availability in your area.
