Professional Referrals

Commissioners & professional referrers

A trusted partner in community care.

CM Community Care works alongside local authorities, commissioners, social workers and other professionals to provide dependable home care built around individual needs.

With established relationships across the region, experienced local teams and robust quality systems, we combine operational reliability with a simple belief: good care should never lose sight of the person receiving it.

Our approach

Dependable delivery. Human care.

We understand that professional partners need confidence that care can be mobilised effectively, delivered consistently and adapted when someone’s circumstances change.

Our teams work closely with professionals to understand individual requirements, establish appropriate care plans and provide ongoing support built around safety, dignity, independence and continuity.

Behind that delivery is the same approach that runs throughout CM Community Care: Care beyond care, looking beyond completing commissioned tasks to understand how good care can support someone’s wider everyday life.

Care services

Support built around individual needs.

CM Community Care’s core services include:

Making a referral

Clear from referral to care.

01. Referral

Send the relevant information to our team so we can understand the individual’s circumstances and support requirements.

02. Review

We’ll review the referral, including the care required, location and our ability to provide appropriate support.

03. Assessment & care planning

Where CM is able to support the package, we’ll work with the relevant people and professionals to understand the individual’s needs, routines and preferences and establish an appropriate care plan.

04. Mobilisation

Our operational teams coordinate the people and support required to begin delivering the agreed package of care.

05. Ongoing review

Once care begins, we continue to monitor delivery and respond appropriately as needs or circumstances change.

Operational reliability

The right people. In the right places.

Reliable care depends on having a workforce that can consistently deliver it.

CM’s operating model is built around local teams, with care runs clustered geographically where possible. This helps reduce unnecessary travel between visits while supporting more consistent, dependable delivery.

We prioritise continuity wherever possible, helping people see familiar carers and allowing our teams to build a stronger understanding of individual needs and routines.

Our carers are recruited for their values and potential, supported through structured training and given opportunities to continue developing their skills.

Quality you can evidence

Strong systems behind every visit.

Our approach to care is supported by quality assurance, internal auditing, safeguarding procedures, care planning, staff training and ongoing supervision.

We monitor the quality of our services, learn from feedback and review care as needs change, helping our teams maintain consistent standards across delivery.

CM Community Care’s regulated services are inspected by the Care Quality Commission (CQC), providing professional partners with independent evidence of service quality.

Working with local authorities

Relationships built over time.

CM Community Care has a long history of working alongside local authorities and professional partners to deliver care within communities across the region.

As the organisation has grown, these relationships have remained an important part of how we operate, combining local knowledge, workforce capacity and established care-delivery systems with an understanding of the requirements of commissioned services.

Our experience also includes working within extra care schemes, supporting people to maintain greater independence while having appropriate care available within their community.
